A amostra deve ser enviada preferencialmente em temperatura ambiente.<\/p><p>AMOSTRA NECESS\u00c1RIA<\/p><p>O esp\u00e9cime deve ser um bloco contendo tecido fixado em formalina e embebido em parafina; Alternativamente podem ser enviadas 3 l\u00e2minas de vidro n\u00e3o coradas, &#8220;carregadas positivamente&#8221;, com cortes de tecido 4 m\u00edcrons de espessura. Uma l\u00e2mina ser\u00e1 corada com hematoxilina e eosina e devolvida.<\/p><p>AMOSTRAS REJEITADAS<\/p><p>Ser\u00e3o rejeitadas os seguintes tipos de amostra enviadas para pesquisa de marcadores celulares: Tecido \u00famido\/congelado; Esfrega\u00e7os de citologia; Tecido fixado com fixador diferente de formalina; Tecido n\u00e3o inclu\u00eddo em parafina; L\u00e2minas n\u00e3o carregadas positivamente.<\/p><\/div>\n\t\t\t\t\t\t\t\t\t<div class=\"elementor-tab-title elementor-tab-mobile-title\" aria-selected=\"false\" data-tab=\"3\" role=\"tab\" tabindex=\"-1\" aria-controls=\"elementor-tab-content-3123\" aria-expanded=\"false\">INTERPRETA\u00c7\u00c3O CL\u00cdNICA<\/div>\n\t\t\t\t\t<div id=\"elementor-tab-content-3123\" class=\"elementor-tab-content elementor-clearfix\" data-tab=\"3\" role=\"tabpanel\" aria-labelledby=\"elementor-tab-title-3123\" tabindex=\"0\" hidden=\"hidden\"><p>INFORMA\u00c7\u00c3O CL\u00cdNICA<\/p><p>O c\u00e2ncer de c\u00f3lon heredit\u00e1rio sem polipose (HNPCC), tamb\u00e9m conhecido como s\u00edndrome de Lynch, \u00e9 uma s\u00edndrome de c\u00e2ncer heredit\u00e1ria autoss\u00f4mica dominante que predisp\u00f5e os indiv\u00edduos ao desenvolvimento de c\u00e2ncer colorretal, endometrial, g\u00e1strico, do trato urin\u00e1rio superior e outros. Indiv\u00edduos com s\u00edndrome de HNPCC\/Lynch t\u00eam uma muta\u00e7\u00e3o da linha germinativa em 1 de v\u00e1rios genes envolvidos no reparo de incompatibilidade de DNA. A maioria das muta\u00e7\u00f5es associadas \u00e0 s\u00edndrome de HNPCC\/Lynch ocorre em MSH2 e MLH1, no entanto, muta\u00e7\u00f5es em MSH6 e PMS2 tamb\u00e9m foram identificadas.<\/p><p>Existem v\u00e1rias estrat\u00e9gias para avaliar indiv\u00edduos cujo hist\u00f3rico pessoal ou familiar de c\u00e2ncer \u00e9 sugestivo de s\u00edndrome de HNPCC\/Lynch. Normalmente, o primeiro passo \u00e9 avaliar os tumores quanto \u00e0s caracter\u00edsticas comuns a indiv\u00edduos com s\u00edndrome de HNPCC\/Lynch, que incluem instabilidade de microssat\u00e9lites (presen\u00e7a de in\u00fameras altera\u00e7\u00f5es em um tipo de DNA repetitivo chamado microssat\u00e9lites) e perda de express\u00e3o proteica de 1 ou mais genes associados \u00e0 s\u00edndrome de HNPCC\/Lynch.<\/p><p>Instabilidade de microssat\u00e9lites (MSI) e imuno-histoqu\u00edmica (IHQ) s\u00e3o comumente interpretadas juntas para avaliar o risco de s\u00edndrome de HNPCC\/Lynch. Altos n\u00edveis de MSI dentro de um tumor s\u00e3o sugestivos de reparo de incompatibilidade de DNA defeituoso, no entanto, essa descoberta n\u00e3o fornece informa\u00e7\u00f5es sobre qual gene est\u00e1 envolvido. IHQ \u00e9 uma estrat\u00e9gia de teste complementar usada para avaliar a express\u00e3o das prote\u00ednas MLH1, MSH2, MSH6 e PMS2 em c\u00e2nceres relacionados \u00e0 s\u00edndrome de HNPCC\/Lynch. A perda de express\u00e3o de 1 ou mais dessas prote\u00ednas dentro do tumor \u00e9 \u00fatil para identificar quais genes correspondentes devem ser alvos para an\u00e1lise de muta\u00e7\u00e3o. Embora MSI e IHQ sejam melhor interpretados juntos, eles tamb\u00e9m est\u00e3o dispon\u00edveis separadamente para acomodar situa\u00e7\u00f5es cl\u00ednicas nas quais h\u00e1 barreiras para realizar esses testes simultaneamente (por exemplo, preocupa\u00e7\u00f5es financeiras, requisitos de esp\u00e9cimes).<\/p><p>A IHQ sozinha pode determinar a reten\u00e7\u00e3o ou perda da express\u00e3o da prote\u00edna MLH1, MSH2, MSH6 e PMS2. Se todas as 4 prote\u00ednas estiverem presentes, a probabilidade de s\u00edndrome HNPCC\/Lynch \u00e9 reduzida, mas n\u00e3o eliminada, porque aproximadamente 5% dos tumores que apresentam MSI tamb\u00e9m t\u00eam express\u00e3o proteica normal para esses 4 genes. A perda de 1 ou mais prote\u00ednas por IHQ sugere reparo de incompatibilidade de DNA defeituoso dentro do tumor e a probabilidade de s\u00edndrome HNPCC\/Lynch \u00e9 aumentada.
