A amostragem deve conter raspagem ampla e repetida de duas faces laterais da vagina.<\/p><p>INFORMA\u00c7\u00c3O NECESS\u00c1RIA<\/p><p>Na an\u00e1lise de dados do Microbioma Vaginal s\u00e3o importante as informa\u00e7\u00f5es cl\u00ednicas da paciente que incluem menopausa, gestante, uso de p\u00edlula, uso de DIU, tratamento hormonal, vaginose sintom\u00e1tica (como dor, corrimento, queima\u00e7\u00e3o, odor, irrita\u00e7\u00e3o na vagina e\/ou disuria), uso recente de antibi\u00f3ticos ou uso de drogas imunossupressoras. Tamb\u00e9m \u00e9 extremamente importante informar se algum exame anterior (at\u00e9 6 meses antes) foi conclusivo para Chlamydia, Neisseria, Ureaplasma, Mycoplasma, Streptococcus e finalmente se a paciente tem hist\u00f3ria de aborto espont\u00e2neo, parto prematuro ou beb\u00ea com baixo peso ao nascer. Necess\u00e1rio pedido m\u00e9dico, mas n\u00e3o consentimento.<\/p><p>INSTRU\u00c7\u00d5ES DE ENVIO<\/p><p>A coleta deste exame deve ser feita antes da coleta de amostras para citologia. <br \/>Para coleta use haste coletora com swab fornecido pela Genoa\/LPCM. <br \/>A Requisi\u00e7\u00e3o tem um adesivo extra, com c\u00f3digo do exame, que deve ser destacado e afixado no tubo de coleta para identifica\u00e7\u00e3o.<\/p><p>Na Requisi\u00e7\u00e3o todas as informa\u00e7\u00f5es s\u00e3o importantes para associa\u00e7\u00e3o com o quadro cl\u00ednico e para cobran\u00e7as do exame. Em seguida coloque o mesmo dentro do saco pl\u00e1stico (3\u00ba Container) e sele com o autoadesivo de sua aba.<\/p><p>AMOSTRA NECESS\u00c1RIA<\/p><p>A haste tem um ponto de fratura e desta maneira mande o tubo coletor com a por\u00e7\u00e3o distal da haste imersa no l\u00edquido conservante. O tubo no laborat\u00f3rio ser\u00e1 submetido no v\u00f3rtex a uma forte agita\u00e7\u00e3o mec\u00e2nica para extrair todos os microrganismos mesmo que estejam pouco representados.<\/p><p>N\u00e3o utilizar esp\u00e9culo para obten\u00e7\u00e3o da amostra para microbioma vaginal pois este pode estar com lubrificantes nas faces externas que interfere na extra\u00e7\u00e3o do DNA.<\/p><p>Uma leve raspagem com o swab deve ser feito na mucosa vaginal, aproximadamente na sua por\u00e7\u00e3o medial. A haste naturalmente ir\u00e1 romper biofilmes (matrizes polim\u00e9ricas), se houverem, e amostrar todas as comunidades bacterianas. Se por estrat\u00e9gia cl\u00ednica (ex. gesta\u00e7\u00e3o) houver necessidade de representar o microbioma do colo uterino fa\u00e7a esta coleta em seguida da coleta vaginal evitando contamina\u00e7\u00e3o com muco vaginal ou lubrificante do esp\u00e9culo.<\/p><p>Sempre que poss\u00edvel registrar o pH vaginal no momento de coleta. O laborat\u00f3rio fornece uma fita de pH 4,5 \u2013 7. A leitura deve ser imediata.<\/p><p>Importante: A paciente deve 48h antes da coleta n\u00e3o ter usado ducha intima (intravaginal), n\u00e3o ter rela\u00e7\u00f5es sexuais, mesmo com preservativo e n\u00e3o utilizar medicamento de uso t\u00f3pico ou cremes vaginais.<\/p><p>AMOSTRAS REJEITADAS<\/p><p>O tubo com l\u00edquido preservante deve ser recebido no laborat\u00f3rio contendo a por\u00e7\u00e3o do swab, destacado da haste coletora. N\u00e3o coletar amostras com outros fixadores como \u00e1lcool, formalde\u00eddo, e outros. Amostra no l\u00edquido preservante enviado no kit Genoa\/LPCM \u00e9 est\u00e1vel por 3 semanas a temperatura ambiente.<\/p><p>Material recebido sem o swab (parte distal da haste coletora) vai ser considerado que o swab foi lavado no l\u00edquido conservante e n\u00famero menor de part\u00edculas celulares e microrganismos estar\u00e3o dispon\u00edveis na amostra. Os microrganismos presentes na vagina, chamados coletivamente de comunidade microbiana vaginal (CMV) s\u00e3o espec\u00edficos tanto em sua composi\u00e7\u00e3o quanto em sua fun\u00e7\u00e3o. Este exame de CMV usa metodologia molecular de NGS (Sequenciamento de Nova Gera\u00e7\u00e3o) para identificar e quantificar n\u00e3o somente bact\u00e9rias como tamb\u00e9m v\u00edrus e fungos que eventualmente estejam presentes na microbiota. Empregando a tecnologia avan\u00e7ada de sequenciamento de alto rendimento (NGS) do 16S rRNA (um segmento conservado do DNA bacteriano e \u00fanico para cada bact\u00e9ria), pode-se sondar a diversidade de micr\u00f3bios presentes no conte\u00fado vaginal.
